Great, I am in network with the above companies. Simply call the customer service number on the back of your card and ask the representative if you have acupuncture coverage, how many visits you are allowed per year, if you have a deductible that applies to acupuncture, and what your copay per visit will be. Once you're ready to schedule, please contact me at (503) 855-9429.
If you don't see your insurance company:
Don't see your company listed above? They may not offer acupuncture coverage or I may not be in network with them. You will be responsible for the cost of treatment in full at the time of service, but I will be happy to provide you with a superbill upon request. A superbill is a receipt that some insurance companies let you submit for full or partial reimbursement for services. Please contact your insurance company for more information re: out-of-network provider reimbursement via superbill.
